We accept most insurances. Before we can begin services, we ask that clients complete our intake forms so that we can verify coverage… the last thing you want is for you to begin services and find out that your insurance will not cover it and be left with a bill.
Many insurance companies cover mental health services. Please check with your insurance carrier to determine your benefits. You will be required to pay insurance copays and deductibles at the time of service. Our office can provide you with more specific information.
We accept HSA/FSA plans, MasterCard, Visa, and Discover cards, as well as cash, check, PayPal and Venmo.
You can also pay your balance through Debra.
If needed, we can offer payment plans. Our office can provide you with more specific information.
Clients are asked to maintain an active credit card on file in order to bill any balances that are due based on insurance copayments and deductibles. If clients do not wish to do so, we may not accept the case, especially for certain types of testing.
In some cases we may ask for a fully refundable fee charged before services are begun. Individuals with Medicaid or Tricare coverage are not subject to this initiation fee as part of our agreement with those specific insurance carriers. This fee will be used to cover any unpaid balance at the end of testing/therapy. If there is no balance, this fee will be refunded to you in whole.
Not that this would ever happen to you …. but we charge a $25 fee for each returned check.
Please come by our office during work hours at your convenience to fill out the intake forms to begin services. If you are not able to stop by, below is a link to download the forms. You may choose to bring the completed forms to the office during work hours, or for your convenience, we have a secure mailbox on the outside gate if you need to drop off the paperwork in the evening or on a weekend.
If you choose to email or fax the paperwork to us, please know that we are not responsible for the security and privacy of your information as we do not have control over Internet and website security.
